New Patients

• Prior to your first appointment with Century Dental, we ask you fill out our Welcome Form which details your personal, dental, and medical histories; you can download or print the Welcome Form, fill it out, bring it with you to your appointment.

• First time patients will also need to fill out our Acknowledgement of Receipt of Notice of Privacy Practices pages.

• You will need to fill out our COVID Disclosure Form each time you attend the office; you can submit your completed COVID Disclosure Form online or bring with you to your appointment.

• You should also bring your insurance cards, claim forms (if applicable), and any referral letters. We accept most insurance plans.

• Don’t forget to bring a list of questions you may have about your treatment.
